Injectable Fillers For Soft Tissue Augmentation - Patients who choose to use a filler to improve the appearance of facial soft tissue are in general looking for improvement in the groove that develops between the cheek and the nose / lip area or they are seeking improvement in a specific scar or scars. For patients who are wishing to see improvement in fine lines and wrinkles there are usually other techniques that work better than soft tissue fillers.
Patients interested in lip augmentation, please refer to that section under surgical procedures. The filler is typically injected into fold or scar in an office setting following adequate numbing of the area. Most patients experience localized swelling for 12 – 24 hours. Bruising can occur but is unusual.

Radiance™ FN is a new injectable
product for lip enlargement and filling of folds and lines. The advantage
of this product is the potential for a long- lasting effect, reported
to be as long as 18 – 24 months in the literature. Radiance™ FN consists
of calcium hydroxylapatite suspended in gel (carbo- xymethylcellulose).
Because calcium hydroxylapatite is a normal constituent of bone, it should not elicit an inflammatory or immune response.
Additionally, the gel carrier doesn't require allergy testing such as required with collagen. Dr. Freeman has found this product to provide excellent results with minimal side effects. The main negative with this product at this time is that the FDA has not approved this product for soft tissue augmentation. The product has been used in Europe for years for soft tissue augmentation.

Restylane is a crystal-clear,
biodegradable NASHA (Non-Animal Stabilized Hyaluronic Acid) gel, which
is injected into the dermis in tiny amounts with a very fine needle. The
result is instantaneous, producing a long-lasting, natural enhancement,
gentle and safe to the skin.
The results of Restylane are reported to last 6 - 8 months and there is no known allergic component.



Dr. Freeman has found this to be an excellent choice for patients who want soft tissue enhancement, especially if they are unsure about how the soft tissue enhancement will improve their looks.

As the only FDA - approved
commercially available dermal fillers made from human collagen, CosmoPlast
offers important advantages. Immediate treatment is possible since it
is made with human collagen and allergic reactions are not a concern.
The filler is formulated with an anesthetic for comfort so no block is
necessary.
Treatment results last up to six months but most patients find a need for more injection within 4 - 5 months.


Dr. Freeman feels that this filler and Restylane are interchangeable depending upon the length of time the patient wishes improvement and the comfort level the patient has with the ingredients.
Facial fat loss, or lipoatrophy,
is the loss of fat beneath the skin, which can result in sunken cheeks,
indentations and hollow eyes. Sculptra is a safe, synthetic and biocompatible
material that is injected below the surface of the skin in the area of
fat loss. It provides a gradual and significant increase in skin thickness,
improving the appearance of folds and sunken areas.
Sculptra is approved by the US FDA for the restoration and/or correction of the signs of facial fat loss in people with human immunodeficiency virus. Sculptra has been safely used outside the US since 1999 in over 150,000 patients under the trade name New-Fill.

There are several
different methods that accomplish non-ablative skin rejuvenation for fine
lines and wrinkles. Dr. Freeman has extensive experience with laser procedures
and feels that the "Smoothbeam" laser from Candela offers the best result.
Dr. Freeman is the first cosmetic laser surgeon in Charlotte area to offer
this technology. Non-ablative refers to procedures that rejuvenate the
deeper part of the skin (known as the dermis) without injuring the upper
part of the skin (known as the epidermis). The reason non-ablative skin
rejuvenation procedures are important is because for the first time they
offer patients improvement in fine lines in wrinkles without the down
time (healing time) associated with laserbrasion or surgery. The treatments
are well tolerated with a topical anesthetic. Each time a patient has
a treatment they will be rebuilding and thickening the collagen and elastin
in their dermis that has been slowly broken down over the years by sun-
exposure, age and perhaps smoking. Over time the fine lines improve.

Treatments are performed every 4 - 6 weeks and each treatment leaves the skin slightly red for less than 30 minutes in the average patient. The treatments are very affordable. Dr. Freeman will take
into account skin color, thickness and texture, and your age to determine
whether or not an individual is a good candidate for laser skin rejuvenation.
Laser skin rejuvenation can be safely performed on any area of the body
that has suffered from sun damage, including hands, face and chest.
Laser rejuvenation is a relatively new technique that Dr. Freeman has pioneered. The technique involves using a laser that targets small abnormally dilated blood vessels, as well as brown discoloration of the skin. These small-dilated blood vessels and brown spots typically are seen in patients over age 30 who have had excessive exposure to the sun over the years. This laser technique generally erases the visible effects of past overexposure to the sun.
The exciting aspect of this technique developed by Dr. Freeman is that it can be completed without a local injection and the laser treatment leaves the skin looking red and irritated for only about 24 hours. A series of treatments is recommended with most patients opting for three to four treatments; occasionally, up to six treatments are performed. These treatments are carried out every four to six weeks and can be done literally over your lunch hour.
There is little risk associated with the procedure. The results have been uniformly impressive with high patient satisfaction. The technique is not a replacement for laser resurfacing in that it will minimally reduce wrinkling; however, if the patient's main complaint is sun-damaged skin, then this is the best procedure.

A tattoo can be removed more
thoroughly and leave less of a mark than ever before. Results are permanent,
and the skin surrounding the tattoo typically is not damaged. Other methods
of tattoo removal (excision or dermabrasion) are more invasive and can
result in scarring. Laser tattoo removal is safer.
Laser tattoo removal is non-invasive and targets the ink used in the tattoo process- not the surrounding skin.

At the Center for Facial Plastic and Laser Surgery, Dr. Freeman uses state-of-the-art lasers to remove tattoos. The treatment is quite safe; however, occasionally a patient may experience a slight lightening of the skin after the tattoo is removed.
Emitted as a pulse of wave light, the laser is directed at the colored areas of a tattoo. When the colored ink particles in the tattoo absorb the laser light, they are broken into tiny fragments. These fragments then are absorbed through the body's natural cleansing mechanism; this usually takes a few weeks. Several treatments usually are required, the number of treatments determined by the depth of the pigment granules and the color of the tattoo. Laser treatments should be performed two to three months apart to allow your body time to remove the maximum amount of tattoo pigment.
While treatments do not require anesthetics, some patients are more comfortable with local numbing provided by an anesthetic cream. Normally this is applied at least 30 minutes before the treatment. Rarely a patient may require a local aesthetic injection. Treatment sessions usually last 10-30 minutes.

Acne can be a difficult condition
to improve. Sometimes topical medications will help or perhaps long term
oral antibiotics. For more difficult or severe acne such as cystic acne
Dermatologists have found Accutane to be helpful.
While these treatments are useful there are those patients who would prefer to have a simple series of laser treatments to dry up their acne. Now they have that option.

Laser based hair removal systems
continue to grow in popularity.
With the promise of never having to shave again, a plethora of companies and devices are attempting to satisfy the demand for safe, painless hair reduction.
Laser based hair removal is based upon the principles of selective photothermolysis - which states that the light energy must be well absorbed by the targeted tissue for increased efficacy and less well absorbed by competing surrounding tissue to prevent thermal injury. In the case of laser hair reduction we are trying to treat the brown pigment in the follicle and not treat the natural pigment of the skin.

Small dilated veins on
the face or elsewhere on the body can usually be safely and painlessly
removed with one laser treatment. In some situations more than one treatment
will be necessary. The treatments can result in a slight bruise that may
take about a week to clear. No injections are necessary.

Physicians have used Botox,
a purified protein produced by the Clostridium botulinum bacteria, safely
for over 10 years. Botox injections relax the muscles that create wrinkles
thereby lessening the appearance of frown lines, laugh lines, forehead
wrinkles and crow's feet. A protein toxin, Botox improves facial wrinkles
that occur with overuse of facial muscles.
At The Center for Facial Plastic and Laser Surgery, Dr. Freeman injects a small amount of Botox into the muscles that are responsible for creating wrinkles. Botox effectively deactivates the muscles that produce wrinkles, causing lines to diminish dramatically or disappear altogether. It may take a few days to notice the wrinkle-smoothing effect after a Botox injection.



Dr. Freeman's Botox treatments are quick and the results usually last three to six months. After that, you may return for a follow-up injection. Dr. Freeman has noted that after several treatments, the effect of Botox appears to last longer, often up to several months longer than the original effect.
For most patients, Dr. Freeman performs a technique that he calls "Botox browlift." This technique in most patients improves the position of the eyebrow while at the same time improving crows feet, forehead wrinkles and wrinkles seen between the eyebrows.
The most common reported complication is a slight, temporary drooping of the eyelid, although Dr Freeman has rarely noted this occurrence in his practice. Botox is used in extremely small amounts and allows for an immediate return to activity, although Dr. Freeman does not recommend that any patient receive a facial or facial massage 48 hours after receiving Botox.
Botox can be used by almost anyone; however, patients who are pregnant or breastfeeding, or who have a neurological disease, should not use Botox.
